The 2020 version of the "Chinese Dry Eye Expert Consensus" defines dry eye as a chronic disease similar to hypertension and diabetes, aiming to position dry eye as a chronic condition requiring long-term care. For patients, dry eye treatment cannot be achieved quickly; it requires long-term treatment in cooperation with a doctor. With appropriate treatment, dry eye symptoms can be completely controlled and alleviated. Therefore, dry eye patients should not be anxious, nor should they have unrealistic expectations; instead, they should actively cooperate with the doctor's treatment plan, using medication correctly and following up regularly to achieve the goal of alleviating dry eye symptoms with the least amount of medication and frequency in the later stages, maintaining eye comfort and improving quality of life.
Dry eye is classified into aqueous deficient, lipid deficient, mucin deficient, tear film dynamics abnormal, and mixed dry eye. Most patients have mixed dry eye, with severe video terminal syndrome as a major cause. For these patients, a single eye drop or treatment plan is insufficient, and combination therapy is necessary. In the early stages of treatment for severe dry eye, multiple eye drops can be used simultaneously, such as eye drops that repair the ocular surface, promote mucin secretion, and have anti-inflammatory effects. Subsequently, the types and frequency of eye drops should be reduced according to the improvement of the condition.
Dry eye is a multifactorial disease, and its core pathological mechanism is inflammation. In the early stages, some corticosteroid eye drops are needed to quickly reduce inflammation, thus alleviating the patient's discomfort such as eye redness, itching, and pain. Subsequently, calcineurin inhibitor cyclosporine eye drops can be used for continued treatment, offering a more lasting and safe anti-inflammatory effect.
